Nomination Petition & Affidavit of Candidacy

**Please contact City Hall if you have any questions. It is imperative that the forms are filled out correctly.

Nomination Petition for Non-Partisan Office – 25 signatures are needed, you will need 2 copies of this sheet – the top of the sheet must be filled out exactly the same on both sheets.

Affidavit of Candidacy – Must be notarized (Needs to be signed in front of the City Clerk)

Paper copies of these forms are available at City Hall & the Glenwood Public Library.

Fully completed forms must be returned to the City Clerk’s office (City Hall – 5 N. Vine Street) from August 28, 2023 to September 21, 2023 at 5:00 p.m. Forms must be filled out completely and the Affidavit of Candidacy needs to be notarized.
